CCI will NOT administer medication unless drugs are in the original prescription bottle, and have been prescribed for that specific animal. CCI staff members are not licensed veterinarians, nor are we licensed veterinarian techs. CCI will carry out veterinarian's written instructions only and must be provided by the veterinarian on the veterinarians' letter head for the specific animal. Arrangement for this type of care must be made IN ADVANCE so that special arrangements can be made for the animal should it have to be quarantined. There will be an additional charge of $20.00 for this specialized care.

CCI will supply  hay, bedding, cage, water, fruits and veggies, and exercise. Anything provided additionally by the owner is at the owners discretion and does not change the price of the boarding.  Owner must provide pellets and enough pellets for this rate or you will be kicked up to the next rate automatically.

Cavy Care Inc will take reasonable care of your animal. If you indicate you do NOT want medical care for your guinea pig while staying with Cavy Care Inc, then you will have to seek boarding at another facility. Cavy Care Inc. will provide medical care for our charges if it is merited for the well being of the animal and not the wallet of the owner. We will not be able to board your guinea pig at this time and ask that you contact another facility.

If for some reason your animal should become ill while in our care that animal will be taken to your veterinarian. If your veterinarian is unavailable, we will be taking your animal to Aurora Animal Hospital for immediate care. Every effort will be made to contact you before going to the vet, but if you are not reachable we will go to the veterinarian. THE OWNER IS FINANCIALLY RESPONSIBLE FOR ALL CHARGES INCURRED AT THE VETERINARIANS. If it is determined that an injury occurred as a result of negligence on the part of CCI, CCI will pay all occurring charges. Regarding veterinarian  fees for illness CCI will pay veterinarian fees until the return of the owner, but will be reimbursed for all out of pocket expenses while the animal is at the veterinarian by the owner upon pick up of that animal from boarding. No animal will be released until the owner has satisfied all expenses. If for any reason your animal should die while in the care of CCI, Aurora Animal Hospital will perform a necropsy to identify what may have caused the animals death at the shelter expense. The owner should pick up the report at the clinic to insure the integrity of that report for all parties involved. All boarding animals are kept separate from shelter animals. They will not be allowed to mix or mingle with shelter animals. That is our policy.

During your animals stay your animal will be feed and watered daily. They will have their cares cleaned every three days or more often if necessary. They will receive fresh veggies/fruits and pelted food. All guinea pigs will get daily interaction and exercise. And if indicated, they will be allowed to have closely supervised visits with like sex during their stays at CCI if other owners have indicate that this is ok. Initial: __________

Payment is made the day of pick up. We cannot accept credit cards, but will accept checks with proper id and cash. We will NOT  take post dated checks for services. If your stay is more than 14 days CCI reserves the right to ask for a deposit. Initial: __________

Call as soon as you get back. Animals left over seven business days past the date of pick up will be considered abandoned. They will be placed for adoption ten business days for that date. It is imperative that you notify CCI if you are going to be late. A certified letter will be sent two days after a no show from the listed pick up date to inform the owner of intent to adopt. Owners will still be responsible for any boarding fees incurred during their animals' stay regardless of abandonment up until the date that the animal is adopted or a letter of surrender is signed by the owner. Owner who show they intend to abandon the animal from the first date of boarding for whatever reason will be charged DOUBLE for stay. Owners will be responsible for all court costs, attorney's fee and any veterinarian cost incurred during the animals stay.  Initial: __________

I certify that I am the owner of record. I have read this agreement (2 pages) and that I understand the contents of this agreement. I also certify that I will pay all fees applicable to the boarding of this animal upon demand.

A deposit of half of the anticipated stay is expected at the time of drop off if you are a new client. The remainder of funds will be collected at the time of pick up. For stays over 7 days we do not charge for the day you drop off or the day you pick up.
